What to bring?
We need to be sure who you are, this means photo ID before we can consult.
Any and all medication you are taking, prescribed and over the counter.
Any blood test or other relevant information that will help the clinician to know what has happened so far.
Bring a list of your questions, minds to go blank in appointments. Bring your concerns so we can work through these with you.
If you are booking for a screening assessment relevant family medical history can be helpful and amounts of exercise, smoking, drug and alcohol use – please be honest, this will help us get the best outcome.
